HEAVENLY PUPPIES-CONDITIONAL-1 YEAR-GENETIC WARRANTY Effective
(08-01-2008) After your regular 30 day genetic warranty (as stated in your contract) has expired, this guarantee then goes into effect on day 31, providing the following conditions are met: 1. You must feed Health Extension Little Bites by Vet’s Choice brand puppy/dog food.
2. You must use Nutra Drops daily.
3. You must use Nuvet Vitamins (powdered or tablets) daily. *You may substitute Vets Choice Vitamins for Nuvet ordered from www.healthextensionhp.com
4. You must keep all vet immunizations, heart worm preventative and well puppy checkups current using a licensed vet only.
5. You must sign/date and return this form to Heavenly Puppies within 30 days of receipt of your puppy, otherwise this warranty is VOID. *As long as you continue your puppy’s recommended food, supplement, vet care & vitamin program according to the directions and labels on the recommended products and the immunization plan set forth by your veterinarian for the first year starting from the 31st day of purchase, this limited warranty will guarantee your puppy against the following congenital conditions. This Warranty is non-transferable and will become void if puppy/dog is sold or given to another person. This warranty applies to pets only and this warranty becomes completely void if puppy is ever used for breeding purposes.
1. Liver Shunt* Must be confirmed by a color flow ultrasound only (no other tests are accepted).
2. Kidney (congenital) defects only.
3. Heart Disease (congenital) defects only.
4. Cancer (Any type). Nuvet Vitamins: Must be ordered from the following sources/using breeder code only. You must tell them you purchased your puppy from Heavenly Puppies to get your special pricing. The auto ship option offers a discount.

Buyer must retain all receipts for food, Nutra Drops, NuVet Vitamins, Health Extension Little Bites food canned & dry, Nutra Drops and Vet visits to prove that you are abiding by this warranty. Proof of purchase (contract) and shipping airway bill must also be presented along with all receipts if any claim is made on this warranty. If puppy was picked up local, the received puppy statement will take the place of the air way bill. The owner is responsible for transportation costs to and from the seller for both returned and replacement puppy. Seller is not responsible for any and all veterinarian expenses accumulated on returned or replaced dog or puppy. The seller is not responsible for damages, physical or emotional to the buyer’s self or possessions and likewise to other individuals. The seller is not obligated to replace a puppy whose problem is a result of its new environment. The prompt return of the pup/dog must be accompanied by a written report from a licensed Vet only along with any test results. In the event of death an autopsy must be performed by a state lab. If an autopsy is not performed this warranty is void. We will not replace a puppy when the puppy has been put to sleep unless both your vet & my vet have agreed on such drastic measures. No cash refunds. No credit refunds. If all conditions outlined in this warranty are met by the buyer, the puppy/dog purchased will be replaced for the congenital defects covered in this warranty with a puppy/dog of equal value. If no puppy of equal value is available within 90 days from activation of a valid claim for this warranty, the purchase price less the cost of airfare, USDA health certificate, travel crate & accessories and travel expenses will be deducted from the cost of the puppy and that amount only will be refunded to the buyer. If the buyer does not want to surrender the puppy/dog, and the defect can be corrected by a veterinarian, the seller will pay ½ of the original purchase price of the puppy/dog less the cost of shipping: airfare, USDA health certificate, travel crate & accessories and travel expense. No guarantees or further warranty applies or refunds/replacements are given if puppy/dog does not survive any treatment procedure. Proof of treatment by a licensed vet must be provided before any refund will be made to the buyer. To submit a claim you must provide a letter from the diagnosing veterinarian, receipts for Health Extension Little Bites dry & canned food, Nutra Drops, NuVet Vitamins and all vet care records, medications, medical tests, a copy of the original purchase/deposit contract, airway bill or receipt of puppy purchased form to the seller. This agreement/warranty represents the total agreement between Heavenly Puppies, the owners and the buyer. No other terms or conditions shall be valid to this sale. I, the undersigned, have read this agreement/warranty and agree to its conditions in full.
I have read and understand the above, I also understand my responsibilities. In the event of any litigation in connection with this warranty, heavenlypuppies.com, Heavenly Puppies and the owners will be paid reasonable attorney fees by the purchaser. Venue and jurisdiction for litigation arising out of or related to this contract shall lie exclusively in Russell County, Phenix City, Alabama.
